Fight or flight vs rest and restore

Your autonomic nervous system has two main gears: sympathetic (fight or flight) and parasympathetic (rest and restore). They're meant to swap in and out across the day. Modern life keeps most people stuck in the first one. That has direct consequences for how your body heals — or doesn't.

What sympathetic dominance actually does

When you're in fight-or-flight mode, your body diverts resources to "deal with the threat right now" tasks: heart rate up, muscle tone up, breathing shallow, digestion down, immune response down, tissue repair down.

That's brilliant when there's an actual threat. It's a disaster when the "threat" is an email inbox at 3pm on a Tuesday and your body has been in that state for fourteen hours.

Why this matters in a physio clinic

Three quiet problems we see all the time:

  • Pain stays elevated. The nervous system is amplifying every signal because it's primed for threat. Mild tissue irritation feels like serious pain.
  • Tissues don't heal as quickly. Repair processes are deprioritised. Injuries that should be resolving in 4 weeks take 12.
  • Muscle tone stays high all the time. The shoulders sit up around the ears. The jaw clenches at night. The lower back never quite lets go. Manual therapy releases it for a few hours, then the nervous system re-tightens everything.

The dry needling thing

Patients sometimes ask why their dry needling or massage relief only lasts a day. The honest answer: because we treated the muscle, and the muscle was tight because the nervous system was telling it to be. The treatment removed the symptom. The driver was upstream.

If your shoulder pain is half-tissue and half-nervous-system, treating only the tissue gets you halfway. The other half is helping your nervous system actually downshift into the second gear.

What the other gear feels like

Parasympathetic dominance is the state your body needs to be in for several hours a day to repair. Slower heart rate, slower breathing (especially extended exhales), reduced muscle tone, active digestion, active immune function, active tissue repair.

You've felt it. The 20 minutes after a really good meal with people you like. The first 15 minutes of decent sleep. The moment after a long exhale at the end of a yoga class. That's the gear. If you don't visit it daily, your body never gets a chance to do the maintenance work it needs to.

How to actually downshift

Things that work, ranked by how much evidence supports them and how easy they are to actually do:

  • Breathing. Extended exhales — breathe out twice as long as you breathe in — for 3-5 minutes. Cheap, free, can be done in a car park before walking into work.
  • Cold exposure. Brief (1-2 minute) cold showers at the end of a normal shower. Hits the vagus nerve and pushes you toward parasympathetic.
  • Slow walking outdoors, no phone. 20 minutes, not a fitness walk. Just walking, looking around.
  • Sleep. Underrated. The single best parasympathetic activity. Most adults are running on too little.

These are not "alternative" things. They're nervous system regulation. They sit alongside any physical treatment we do, and for some people they make the difference between treatment that sticks and treatment that doesn't.
